Requirements for Owners

Please fill out the New Utility Contract Form and return by:

In Person at: 1090 E. Union St. San Luis, AZ 85349

Owners must also provide the following information along with all other required documents preferably by email:

• Proof of property / ownership: Proof must include property address and owner / applicant’s name
• Copy of property owner(s)’ valid identification & Social Security Number*
• Mailing address
• Telephone number
• Email address if you would prefer to be contacted by email
• Signed contract application

Fees for Owners

The city uses a 3rd party collection agency to collect on delinquent accounts. The account holder Social Security Number is required for the purpose of collecting on a debt and/or reporting to the State Unclaimed Property.

Requirements for Tenants

Please fill out the New Utility Contract Form. Tenants must provide the following information along with all other required documents either by fax or email:

• Copy of rent receipt and/or rental contract which must include the property address, date, applicant's name, and the property owner’s signature
• Copy of applicant(s)’ valid identification and Social Security Number*
• Mailing address
• Telephone number
• Email address if you would prefer to be contacted by email
• Sign contract application
