The City offers the following two Medical Insurance Plan Options:
- Plan 1: BCBSAZ PPO Network
- Uses the Blue Cross/Blue Shield network in Arizona
- No referrals are required
- Prior Authorization for complex imaging, out-patient services and hospitalizations
- International travel covered as an emergency
- Plan 2: Siarmed Network
- Mexico-only Physician network - limited to Siarmed network for both medical and dental providers
- Requires selection of a Primary Care Physician
- Referrals to Specialists or for Specialized imaging are required
- Members must present a photo ID to receive medical service
